If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
- Full Pay Discount: We offer a 10% accounting courtesy for all treatment that is paid in full (cash or check) at the time of service.
- Major Service - 2 payment option: We offer a two-payment option for Crown, Bridge, partial and Denture treatment. We ask that you pay one-half of your co-payment/portion at the first appointment and the second half within 2 weeks. We do require that you leave a credit card # on file for the second payment or a post-dated check for the remainder amount due.
- Care Credit: For our patients who want to make monthly payments. We offer short and long term financing through Care Credit. **Our financial coordinator will gladly assist you with the application process**
- Co-Payment: Our goal is to help you maximize your dental insurance benefits. We have verified your insurance coverage and are required to collect from you the amount they notify us is due from you
